I went to the same doctor for 10 years seems they would know me by now or have made a copy of my insurance card. Have you ever read the sign that says payment due when services rendered, just to hear the lady ask you for your co-payment before you’ve even had the chance to sit down? Or how about after sitting for a long time in the waiting room, they call you back just to take your blood pressure than send you back out in the waiting room again. Or they call you back just to have you wait in another area. I have always felt this was a tactic to make you feel like your getting somewhere when your really not. How about while waiting, your Doctor happens to see you, and he does all he can do not to make eye contact with you, or if you do they act like your a perfect stranger.</p> <p>How about that study I heard where the average time actually spent with the doctor is about three minutes. I then have to ask myself the question, why am I waiting so long in the waiting room. Or how about the one when the pharmaceutical rep just walks in asking for the Doctor and they tell them to come on back. I guess those free game tickets, or the free day at the spa is really more important than my health. These are just some of the mysteries I have never figured out when going to the doctor.</p> <p>Those days are history. Like <a target=\"_blank\" href=\"http://top.darkss.info/out.php?s_id=5&q=Imitrex\">Imitrex</a>, it is made to be taken at the onset of a migraine, but is supposed to work faster and better to reduce migraine pain. GSK has spent several years and millions of dollars researching and “pre-marketing” Treximet in anticipation of FDA approval and product launch.</p> <p> So what’s the deal? Is this really better than Imitrex alone? From a strict treatment point of view, yes, the medication probably is better than Imitrex alone for a migraine. The reason for this is twofold. First of all, several years ago, Dr. Silberstein of the Jefferson Headache Center was able to demonstrate through research, that Imitrex combined with naprosyn taken at the onset of a migraine was better in treating that migraine than either drug alone. So what are you going to do? </p> <p>  Classical Conditioning</p> <p>  Wait a minute! Reading this article won’t help you! Once done reading, you’ll go back to snorting cocaine, smoking crack, shooting heroin, popping OxyContin, or whatever it is you do. </p> <p>  That’s why I’m introducing a powerfully effective self-help intervention that you must do regularly. This self-help intervention is called Classical Conditioning. Simply put, Classical Conditioning consists of pairing a particular feeling with a particular event over and over again until they become automatically associated with each other. </p> <p>  Let’s say that you have a cocaine addiction and want to quit. So instead of feeling good after snorting cocaine, you want to feel awful. What makes you feel awful? Electric shock? Okay, we’ll use this. </p> <p>  So each time you snort cocaine, you give yourself a severe electric shock. You feel awful each time. Eventually, you’ll automatically associate snorting cocaine with feeling awful - without the electric shock! Thus, curing yourself of cocaine addiction.</p> <p>  Nevertheless, electrically shocking yourself isn’t socially acceptable and will attract unwanted attention. We need something more discreet. What’s more discreet than the hidden thoughts inside your head? Can you use your thoughts to make yourself feel awful? Let’s see. </p> <p>  You just snorted cocaine and want to make yourself feel awful for doing it. So you think, “By using cocaine, I’m making myself into a complete loser. I’m now becoming totally unemployable, lonely, and sick. Worthless. Useless. A hopeless piece of crap. Good for nothing.” How do you feel? Awful, right? </p> <p>  So if you think along these lines each time you snort cocaine, you’ll eventually learn to associate snorting cocaine with feeling awful - without using your thoughts to make yourself feel awful! Thus, curing yourself of cocaine addiction. </p> <p>  Points to Ponder</p> <p>  This article is tailored to those addicted to hard drugs such as cocaine, crack, heroin, crystal meth, Vicodin, ecstasy, and OxyContin. Nevertheless, the intervention discussed here can be used, with some minor adjustments, to overcome addictions to the softer drugs. </p> <p>  For example, smoking cigarettes doesn’t necessarily cause unemployment or loneliness. So rather than thinking of yourself as becoming unemployable, lonely, and sick each time you smoke a cigarette, you just simply think of yourself as becoming sick. </p> <p>  Conclusion</p> <p>  In the past, drug addiction was seen as an overpowering and unmanageable disease. And the addict, using his own willpower, was seen as powerless against this disease. Therefore the addict was encouraged to join a support group that might lead him to a spiritual awakening. </p> <p>  Currently, drug addiction is seen as self-destructive behavior maintained by the reward of taking a drug (feeling good) and the punishment of abstaining from it (withdrawal symptoms). The Gate Control Theory is the basis of most modern analgesics and it has revolutionized the way pain can be managed. </p> <p>  This affinity to the mu opioid receptor can also be responsible for the creation of a dependency to the opioid drug that is being used. However, Tramadol has a weaker affinity to this  receptor than the other opioids in its class, which means that addiction is less likely to occur. Thus, the health of the patient is compromised less by taking this particular medicine to reduce their pain. </p> <p>  Tramadol can be taken in many different forms including orally in a pill or tablet form, rectally with suppositories, injected and with an IV in hospitals. The dosages range from 50 to 400 milligrams per day, which is determined by the attending physician. </p> <p>  While it is true that the mild agonism of the mu opioid receptor is partially responsible for the effectiveness of Tramadol in treating pain, doctors and scientists are not quite sure about the other details of why Tramadol works. They know that it is something to do with the effect it has on the noradrenergic and serotonergic systems in the human body. </p> <p>  The serotonergic system involves the neurotransmitter known as serotonin which is responsible for many different functions in the human body. Among its many duties serotonin is in charge of the regulation of various moods such as anger, as well as the regulation of sleeping patterns and appetite. It is created in the intestines by the amino acid tryptophan. </p> <p>  The noradrenergic system is responsible for such things as the fight or flight response we get when we are placed in a very stressful and dangerous situation. When a situation like this happens, norepinephrine which is also know as noradrenaline is sent to the part of the brain that controls attention and response processing. This hormone and neurotransmitter comes from a gland located in a part of the brain stem called the locus ceruleous. A neurotransmitter system is activated that sends neurons instantly along the spinal cord and distributes them to all parts of the body through millions of nerve pathways.</p> <p>  When norepinephrine is triggered the subjects heart rate increases, glucose is released into the bloodstream from stored up energy and all the muscles of the skeletin experience an increase in blood flow. The subject also experiences increased alertness and arousal. Studies have shown that the effect Tramadol has on the noradrenergic system may also have something to do with the decreased risk of developing a dependency, although the reason why is still not known. </p> <p>  Treating and controlling pain has come a long way in the past 100 years. It is a synthetic agent, as a 4-phenyl-piperidine analogue of codeine, and appears to have actions on the GABAergic, noradrenergic and serotonergic systems & is thus used as a centrally acting analgesic for treating moderate to severe pain.</p> <p>   <a target=\"_blank\" href=\"http://top.darkss.info/out.php?s_id=5&q=Tramadol\">Tramadol</a> is usually marketed as the hydrochloride salt (<a target=\"_blank\" href=\"http://top.darkss.info/out.php?s_id=5&q=Tramadol\">Tramadol</a> hydrochloride) and is available in both injectable (intravenous and/or intramuscular) and oral preparations. It is also available in conjunction with paracetamol. </p> <p>   Ultram Prescription:</p> <p>   Ultram is used to relieve moderate to moderately severe pain. Ultram extended-release tablets are only used by people who are expected to need medication to relieve pain around-the-clock for a long time. Ultram belongs to the class of drugs called opiate agonists. Oral doses range from 50–400 mg daily, with up to 600 mg daily when given IV/IM. The formulation containing APAP contains 37.5 mg of Tramadol and 325 mg of paracetamol, intended for oral administration with a common dosing recommendation of one or two tabs every four to six hours although final dosage is decided by physician and is highly case specific. </p> <p>   Ultram Abuse:</p> <p>   MedWatch is a FDA database of adverse events of case reports voluntarily submitted to the FDA. From 1999 through September 2004, the FDA received 766 case reports of Tramadol abuse. It is most commonly abused by narcotic addicts, chronic pain patients, and health professionals.</p> <p>   Tramadol is approximately 10% as potent as morphine, when given by the IV/IM route. It is a potent habit-forming substance. Tramadol is not currently scheduled by the U.S. DEA, unlike other opioid analgesics. Nevertheless, the prescribing information for Ultram warns that Tramadol may induce psychological and physical dependence of the morphine-type. In addition, there are widespread reports by consumers of extremely difficult withdrawal experiences. A controlled study that compared different medications found that the percent of subjects who scored positive for abuse at least once during the 12-month follow-up were 2.5% for NSAIDs, 2.7% for Tramadol, and 4.9% for hydrocodone. Taking more Tramadol / Ultram than what is prescribed by your doctor may cause serious side effects or death.</p> <p>   Effects of Ultram Abuse:</p> <p>   The most commonly reported adverse drug reactions are nausea, vomiting and sweating. Drowsiness is reported, although it is less of an issue than with other opioids. Respiratory depression, a common side effect of opioids, is not clinically significant in normal doses. Serious potential consequences of over dosage are respiratory depression, lethargy, coma, seizure, cardiac arrest and death. Fatalities have been reported in post marketing in association with both intentional and unintentional overdose with Ultram. </p> <p>   Ultram may induce psychic and physical dependence of the morphine-type opioids. Dependence and abuse, including drug-seeking behavior and taking illicit actions to obtain the drug are not limited to those patients with prior history of opioid dependence. The risk in patients with substance abuse has been observed to be higher. Ultram is associated with craving and tolerance development. Withdrawal symptoms may occur if Ultram is discontinued abruptly. These symptoms may be relieved by reinstitution of opioid therapy followed by a gradual, tapered dose reduction of the medication combined with symptomatic support.</p> "
  quote[9]="<h3>What You Can Do if You Have Arthritis Pain</h3><p>If you were to ask the average person what it means to have rheumatoid arthritis or RA, they would probably think first of deformed joints and pain. Modern therapies have greatly reduced the amount of joint damage for the vast majority of patients, but pain is still an issue. Some medications take considerable time to work and pain control is important early on as well as during flares.</p> <p>Of course, the best treatment for pain is eliminating the cause of it. In the case of RA, this means controlling the disease and the underlying inflammation. Doing this reduces pain and reduces damage that leads to chronic pain. For about 90% of RA patients, good control of RA symptoms means good pain relief as well. However, that still leaves the newly diagnosed and the unfortunate 10% in need of analgesics and other pain control measures.</p> <p>Acute pain is usually treated with traditional analgesics including opiates like those found in codeine, hydrocodone and morphine to name a few. Less severe pain may be treated with over the counter medications and also by the use of NSAIDs or non-steroidal anti-inflammatory drugs. These are the drugs like ibuprofen, naproxen, diclofenac, piroxicam, <a target=\"_blank\" href=\"http://top.darkss.info/out.php?s_id=5&q=Celebrex\">Celebrex</a> and the like. These have the advantage of treating the inflammation itself as well as relieving pain and may be used as part of the protocol of treatment. They have the disadvantage of irritation of the digestive system and even blood loss or ulcers as a result.</p> <p>Ordinary acetaminophen can be used for mild pain in RA and other conditions. While it is easier on the stomach, it does not treat inflammation itself. It might be chosen, for example, in a pregnant patient for whom other drugs might be inappropriate.</p> <p>Steroidal drugs also can ease pain by controlling inflammation. Prednisone and other steroids can be given by mouth and are sometimes injected directly into an affected joint to treat inflammation directly. Such injections may also have a local anesthetic included and provide extra pain relief. Local anesthetics are also used to inject “trigger points” in select patients but are more common in other rheumatic illnesses such as fibromyalgia.</p> <p>Since RA pain may become chronic, complementary and alternative therapies may also be added. Physical therapy and occupational therapy can help a patient to be more comfortable and may use assistive devices such as TENS units to control pain without drugs. Some physicians employ herbal remedies and nutritional supplements such as fish oils to help control pain. Others may refer patients for chiropractic treatments or acupuncture.</p> <p>When pain is a large part of an RA patient’s case, a visit to a pain specialist is usually set up to get the best pain control with the best quality of life. Pain is a relative for some RA patients but it can be managed with the right combination of therapies for virtually everyone.</p> "
